@AbraarKaran 3/ In times of low community incidence, screening all asymptomatic may put strain on lab capacity for small benefit. If symptomatic or in shared rooms, would definitely advocate for testing in this scenario though as outlined above. Benefit of hospital is you can identify quickly

@AbraarKaran 1/ Also, the move recently has been away from screening asymptomatic patients who are being admitted to the hospital. @AbraarKaran 10/ LA County Public Health has been very creative and has created smart programs through CDC support to incentivize SNFs to vaccinate more residents which has been working. More public health departments need to step in and get the job done here.

@AbraarKaran 9/ I have visited a few SNFs recently in which bivalent booster uptake was very low. There were misunderstandings about the role of vaccinations even for elderly high risk folks, and there isn't adequate incentive (IMO) for SNFs to close these gaps quickly

@AbraarKaran 7/ But for now, much work remains. As @AshishKJha46 & WH have been pushing for months, we need to close the bivalent booster gap. Recent CDC MMWR survey suggests people that didn't get it either weren't aware or didn't know if they were eligible. This is a public health issue.

@AbraarKaran 6/ Furthermore, CDC tracks excess mortality data for the pandemic. Most of this has been either due to Covid19 directly, or to issues related to the virus; some also related to other effects from the pandemic itself. Over time, maybe this will lean further away from Covid itself

@AbraarKaran @awgaffney I agree that one can overcome hesitancy; recent conversations in SNFs I visited were eye-opening with regard to reasons both staff and residents were not getting the bivalent. But haven't seen larger-scale surveys as to what is going on nationally in this population.

@AbraarKaran One of the arguments I heard in a skilled nursing facility (among staff w/o boosters) was that they got initial vaccines but still got Covid19 We did not do a good job getting across the key purpose and benefit of vaccines, which was to prevent severe disease, not all infections

@AbraarKaran 2/ ultimately I agree w your diagnosis- there has been a deliberate frame shift; efforts to normalize current harms, or at least rely on vertical interventions not easily accessible to all (Paxlovid) or perhaps w/o addressing concerns to implement effectively (booster shots)

@AbraarKaran 5/ also interestingly the family member had almost no symptoms at all during the infection a few months ago while remaining family had symptoms/were ill. Now during this second infection they are symptomatic— fatigued, coughing, headaches etc

@AbraarKaran 4/ also had rapid tests on hand to use as soon as symptoms started; and pulse ox to check overnight. So far no one else has tested positive or had symptoms Sharing this to say- many steps need to happen quickly w/ many resources needed to prevent spread/ treat people at home

@AbraarKaran 3/ we have space to have index isolate separately w/ separate bathroom. Many if not most don’t have this, esp during holidays when other family may be visiting too Missed days of work; hopefully quick recovery thanks to antivirals but pragmatic challenges here

@AbraarKaran 2/ take care of family member. Very clear though that if I were not here, this process would not have been as easy Had to leave middle of the day, 1.5 hour wait at pharmacy (Kaiser); able to do it but many jobs would preclude this flexibility

@AbraarKaran 1/ Yet again family member sick w/ #covid19 few months after initial infection Was able to quickly get them Paxlovid (through colleagues who work in that system— wait for a clinic callback otherwise up to 2 days) Able to use GVS N99 I had in supply to protect self while helping

@AbraarKaran @wsbgnl Yea I think the efforts would need to be far more than messaging. And not sure federal government can set up any mask related policy that is enforceable on states or counties. Lack of protests (to me) reflects perhaps more deeply where these issues are in terms of ppl priorities
