A physician or non-physician practitioner who did not establish a User ID and password in NPPES but applied for an NPI using the paper application can still establish a User ID and password in NPPES by going to NPPES at https://nppes.cms.hhs.gov/nppes. The NPI Enumerator at 1-800-465-3203 can provide assistance.
How do I access my NPI?
CMS has created an online NPI registry(nppes.cms.hhs.gov) where a physician, other health care provider, or organization can access NPI information. This is part of the same National Plan and Provider Enumeration System (NPPES) web site on which NPI registry is completed.
How do I reset my NPI password?
'Forgot User ID or Password? ' Button:
- If you enter an incorrect User ID and Password combination three times, your User ID will be disabled.
- Please contact the NPI Enumerator at 1-800-465-3203 if your account is disabled.
How do I update my NPI information?
Updates can be made by mailing a Paper Application/Update Form available for download by accessing https://nppes.cms.hhs.gov or by contacting the NPI Enumerator and requesting one via mail. In Section 1A Reason For Submittal of this Form, select the Change of Information box.
