How do I activate my Flores benefits card?
Your Flores Benefits Card will be mailed upon your enrollment in an eligible benefit plan. No activation is required, but you should review the Cardholder Agreement included in this mailing, and then sign the back of your card.

If you are unable to locate this number, your HR department may be able to assist you, or you may also contact Flores at 800-840-7684 during our normal business hours - 8:30 am to 5:00 pm EST.
What is Flores mobile app?
Flores Mobile (formerly e-Receipt) App provides a single access point for participants to upload debit card documentation/claims while providing real time access to consumer driven account data whenever and wherever through our fully responsive website.
What is my Flores PID?
Your unique Participant ID number was mailed to your home address when you signed up for the plan. The PID can also be found on: Rejection Notice – your PID is located in the upper right hand corner below the bar code. Reimbursements – Your PID appears on you reimbursement check stubs or direct deposit confirmations.
